That whenever any physician or other person practicing as such siinll have the charge of, or shall be called to visit any person within rhr li mits of this city who sball be sick or infected with the Hinall pox, cholera, scarlet fever, mensles, or any othor infectious or contagious disease or (linease dangernus to pub ie ealth, he orsheshall lmmediatedly give notice thereof to the chainnan of the Board of Health of this city, or the Health-ofilcer of said Board, together with the name and residence of patiënt; and that whenever any physician or other person practicing as such, or any midwife shall be attending any person wbo may die f rom any cauae such physician or other peraon, or midwife, 80 attending shall withia twenty-four (24) hours after such death give nolice tnereof, witb cause of death, to the said chairman of the said Board of Health or to the said Healt h-oftlcer. Whenever any householder shall know or be informed that any person within bis or her place of residence dies who bas not been attended by a physician, or that any person is taken Kick, or ia infected with small poz, cholera, scarlet fever, meaales, or auy otber infectious or contagious disease, or disea6es dangerous to public health he or she shall immedtatel; give uotice thereof to the chairman of the Board of Health of thls city, or the Health-offlcer of said Board, together with lus or her pLice of resideoce. and name if known of such patiënt, or deccased person, and cause of death if it shall be known.
